Health Services and Counseling Services are located on the 2nd floor of the Strauch Family Student Center.

9 a.m. to 5 p.m.

Appointments

Appointments for Health Services are required to reduced potential exposure to illness. 

To make an appointment for Health Services or Counseling Services, students can contact the office between 8 a.m. and 4 p.m. at 802-387-1636.

Dental Savings Program

UNI-CARE Dental Savings Program

Students enrolled in the Gallagher Student Health Insurance Plan have access to a discount dental plan that is available through UNI-CARE. This is a dental insurance plan; there is no waiting period.

The UNI-CARE savings program provides you with a wide range of dental discount services. Savings vary but can be as high as 50% depending on the type of service received.

· Download your membership card and present it to the provider prior to treatment to ensure proper discount is applied

Vision Savings Program

Coast to Coast Vision Savings Program

Students enrolled in the Gallagher Student Health Insurance Plan have access to a discount vision plan that is available through Coast to Coast vision. This is not a vision insurance plan; there is no waiting period.

Coast to Coast’s network has over 20,000 independent providers and retail stores nationwide, including For Eyes Optical, Lens Crafters, Pearle Vision, Target Optical, JC Penney Optical and Visionworks locations. Students save 10% to 60% off eyeglasses, contact lenses and other retail eyewear items, as well as 10% to 30% off eye examinations at any participating Coast to Coast Vision provider and 40% to 50% off the national average on LASIK.

How can I submit a claim for a prescription paid for out-of-pocket?

If you paid out-of-pocket for a covered presecription medication at a participating pharmacy without using your pharmacy benefits, you can submit your claim for reimbursement.

Is Mail Order Service Available?

If your ID card lists Wellfleet Rx/KPP with BIN 012882 and your plan covers medication through mail order, you have access to PPS, a full-service mail-order pharmacy. Refer to your plan document to determine if your plan covers medication through mail order.

If your ID card lists Wellfleet Rx/ESI with BIN 003858 and your plan covers medication through mail order, you have access to Express Scripts Pharmacy, a full-service mail-order pharmacy. Refer to your plan document to determine if your plan covers medications through mail order.

Claim Form

Providers will bill the claims company directly. In the event that you paid for services and need to be reimbursed, mail the original invoice (keep a copy), proof of payment, and a copy of your insurance ID card to Wellfleet. Please retain a copy of all mailed documentation for your records. Approximately 30 business days are required to process a reimbursement for medical claim.
