How can I submit a claim for a prescription paid for out-of-pocket?
If you paid out-of-pocket for a covered presecription medication at a participating pharmacy without using your pharmacy benefits, you can submit your claim for reimbursement.

Is Mail Order Service Available?
If your ID card lists Wellfleet Rx/KPP with BIN 012882 and your plan covers medication through mail order, you have access to PPS, a full-service mail-order pharmacy. Refer to your plan document to determine if your plan covers medication through mail order.
If your ID card lists Wellfleet Rx/ESI with BIN 003858 and your plan covers medication through mail order, you have access to Express Scripts Pharmacy, a full-service mail-order pharmacy. Refer to your plan document to determine if your plan covers medications through mail order.

Claim Form
Providers will bill the claims company directly. In the event that you paid for services and need to be reimbursed, mail the original invoice (keep a copy), proof of payment, and a copy of your insurance ID card to Wellfleet. Please retain a copy of all mailed documentation for your records. Approximately 30 business days are required to process a reimbursement for medical claim.
